Continuum of Care Workgroup
Our Focus:
Improving access to a continuum of care for individuals affected by brain injuries and other brain health issues. This includes improving coordination of care among medical providers, educators, the community, and families.

Fall Resource Fair
We hosted a resource fair for individuals impacted by brain injury on October 22, 2024. The event was held at Goodwill Easterseals Miami Valley Main Campus, and featured the following providers:

Health & Lifestyle Program (HeLP)
We are creating the pilot program HeLP in partnership with local YMCAs and the University of Dayton. This program is a place for clinicians to refer traumatic brain injury patients who may benefit from help with community reintegration, involving support from physical therapy graduate students in the YMCA setting.
Brain Injury Resource Packs
In partnership with the Play4Payne Foundation, we have created Resource Packs to be given to brain injury suvivors by our local hospitals at discharge. These backpacks have a binder and notebook to help the survivors keep track of important information, a reusable water bottle, UNO cards, and a practical guide to navigating brain injury, with pamphlets and a contact form to help us connect with them for additional support.
Social Connections for Families Affected by TBI
The Play4Payne Foundation runs a monthly social for families affected by TBI. Part of our work involves creating and promoting events like these.
