After logging into myhealth.udayton.edu using your Porches username and password, click on “View My Health Requirements.” You will click on the box labeled “update” for each listed health requirement. You will be asked to enter the vaccination dates (mm/dd/yyyy). Failure to enter the immunization dates will result in non-compliance with the medical clearance requirements. Click the “upload immunization record” tab (located in the lower section of the required immunization box) to upload supporting documents.
Acceptable supporting documents include:
- a healthcare provider-signed document with vaccine names and dates
- immunization records from your previous school on the school's letterhead
- an official immunization record from your patient portal (like MyChart or similar)
- lab results showing your immunity
If you have trouble uploading records, email healthcenter@udayton.edu or call 937-229-3131 and press option 2.
After you upload your information, UD Student Health Center staff will review it within two weeks. You can check Porches to see if you are marked compliant. If you are missing required vaccines or information, you will get an email with instructions. Monitor your status on Porches.
If you don't have your vaccine records, check with your pediatrician or family physician, high school or previous college to see if they have a copy. If you are unable to obtain records, you can either opt to get the vaccines again or you can get blood tests to prove immunity. If you are unable to meet the deadline, upload as much information as possible. We will review it and send further instructions via secure message or email if needed.
