Health and Vaccine Requirements

After logging into myhealth.udayton.edu using your Porches username and password, click on “View My Health Requirements.” You will click on the box labeled “update” for each listed health requirement. You will be asked to enter the vaccination dates (mm/dd/yyyy). Failure to enter the immunization dates will result in non-compliance with the medical clearance requirements. Click the “upload immunization record” tab (located in the lower section of the required immunization box) to upload supporting documents. 

Acceptable supporting documents include: 

• a healthcare provider-signed document with vaccine names and dates
• immunization records from your previous school on the school's letterhead
• an official immunization record from your patient portal (like MyChart or similar)
• lab results showing your immunity

If you have trouble uploading records, email healthcenter@udayton.edu or call 937-229-3131 and press option 2.

After you upload your information, UD Student Health Center staff will review it within two weeks. You can check Porches to see if you are marked compliant. If you are missing required vaccines or information, you will get an email with instructions. Monitor your status on Porches.

If you don't have your vaccine records, check with your pediatrician or family physician, high school or previous college to see if they have a copy. If you are unable to obtain records, you can either opt to get the vaccines again or you can get blood tests to prove immunity. If you are unable to meet the deadline, upload as much information as possible. We will review it and send further instructions via secure message or email if needed.

• June 7, or one week after confirming enrollment for fall semester if you enroll after June 7. 
• Jan. 2 if enrolling for spring semester 

A $50 noncompliance charge will be assessed Aug. 1 for the fall semester or on the first day of classes for the spring semester, if you do not meet the deadline. A registration hold will be placed on your account if you remain noncompliant six weeks after the start of the semester.

If you answer YES to any of the questions, you MUST get a TB blood test (Quantiferon Gold or T-Spot) done by a U.S. lab within six months before arriving on campus, and attach results at myhealth.udayton.edu. A positive or borderline Quantiferon Gold or T-Spot requires a chest X-ray, laboratory report and/or TB treatment that can be uploaded at myhealth.udayton.edu. Students may be tested by the UD Student Health Center upon arrival.

Students who have a positive TB test and a negative chest X-ray are required to discuss preventive treatment with a physician. If you decline treatment, you will be asked to sign a refusal form and have an annual evaluation for TB. It is the responsibility of the student to pay for any testing and/or treatment.

All incoming students including graduate, law and international students born in 1957 or later are required to submit evidence of having been adequately immunized against measles (rubeola), mumps and rubella (MMR) at myhealth.udayton.edu. Students needing to fill out the form in Arabic, Chinese or Spanish and send them to the UD Student Health Center can do so using the aforementioned links.

Students who request an exemption for MMR vaccine requirement by emailing healthcenter@udayton.edu may be REQUIRED to leave the campus in the event of a mumps, measles or rubella case. The student also will be required to sign a release stating they understand the risks of non-immunization and hold harmless the University of Dayton and the Ohio Department of Health (including appropriate public health authorities) from any responsibility for adverse consequences of their refusal to be immunized against measles, mumps and rubella, including but not limited to, contracting one or more of these diseases, financial loss and inability to complete course requirements.

The following vaccinations are strongly recommended:

• COVID-19 (including booster)
• Hepatitis A
• Hepatitis B
• Influenza
• Meningitis
• Tetanus-diphtheria (with or without acellular pertussis/whooping cough).

While the strongly recommended vaccinations are voluntary, the state of Ohio requires students living on campus to attest they understand the risks if they are not vaccinated against meningitis and hepatitis B by completing the meningitis and hepatitis B exemption forms located at myhealth.udayton.edu. Students needing to fill out the form in Arabic, Chinese or Spanish and send them to the UD Student Health Center can do so using the aforementioned links.
