Am I required to submit health information before coming to UD?
The University requires all students to provide immunization information and complete a tuberculosis (TB) questionnaire before starting classes. A $50 noncompliance charge will be assessed if the requirements are not met by the submission deadline. A registration hold will be placed on your account if you remain noncompliant six weeks after the start of the semester.

How can I submit my health requirements and insurance card?
- Log onto myhealth.udayton.edu using your UD username and password.
- Click on "View My Health Requirements."
- Click on each box labeled "Update" to complete each health requirement. You will be asked to enter vaccination dates (mm/dd/yyyy) - failure to enter the immunization dates will result in non-compliance with the University health requirements.
- Click on the "Upload Immunization Record" to upload supporting documents.
- You can submit a copy of your health insurance card within the UD Student Health Center portal. Click on the "Insurance Card" tab on the left side of the screen, then click "Add New Card".
Do I need an appointment?
Yes. Appointments can be made by calling 937-229-3131. Please give a brief description of what you need to be seen for, so you will be scheduled for an adequate appointment time with the appropriate provider. You will need to arrive 15 minutes befoe your appointment time. Please call or go to our student portal at myhealth.udayton.edu to cancel your appointment at least one hour before your visit. Walk-ins are for emergencies only.
What happens if I miss my scheduled appointment?
- If you are scheduled with a physician and do not cancel at least one hour prior to appointment, you will receive a secure message informing you there will be a $50 charge on your student account.
- If for any reason you can't make your appointment, please either call us at least an hour before or go online 24/7 to myhealth.udayton.edu to cancel.
How do I pay for services?
There is no charge for consultations with the nurse or physician for undergraduates, law, DPT and PA students. Graduate and IEP students are charged for each UD Student Heath Center visit.
All students are charged for:
- Medicines
- Laboratory testing
- Vaccines
- X-ray examinations
- Certain procedures performed by medical staff
- Miscellaneous medical supplies (e.g., ankle braces, dressing supplies)
All charges are placed on the student's account at the Office of Student Accounts. Insurance companies are not billed directly; however, students may request an itemized bill. Direct pay may be requested in certain situations.
All students should have a copy of their insurance card on file with the UD Student Health Center in case lab work, outside prescriptions or referrals are needed.
Can my parents receive information about my visit?
Student Health Center visits are strictly confidential. Exceptions are made if a clear and imminent danger exists, or with the consent of students.
What if I am younger than 18?
Currently enrolled University of Dayton students 15 years of age or older are considered mature minors at the UD Student Health Center. The mature minor status allows for minor students to receive medical care and/or treatment without parental/guardian consent.
Do you write medical excuses for missed classes?
No. If the professor requests confirmation of illness, the professor may submit a request on the UD Student Health Center website. Only verification dates will be given. Students must work with their professors to make up missed work.
Can I receive allergy injections?
Yes, as long as you have received allergy injection(s) in your allergist's office without significant reactions. Before any injections can be given at UD, the Student Health Center allergy immunotherapy forms must be completed and received along with allergy serum. The student is required to have an initial and annual physician visit for this service.
Can you fill my prescription?

The UD Student Health Center does not have an outpatient pharmacy. However, for your convenience, we do carry some commonly prescribed medications and make them available to students if ordered by our physicians. If the Student Health Center does not carry the medication, or if the student wants to use their insurance, we can order it from Walgreens (located on Wayne Ave.) and have it delivered to the UD Student Health Center. Students can pick up their prescriptions during regular business hours.

For prescriptions written by outside doctors, students may drop the prescription off at the UD Student Health Center and it will be sent to Walgreens along with a copy of their insurance card. Please allow one to two business days for delivery of medication to the UD Student Health Center. Before prescriptions will be delivered, arrangements for payment needs to be made directly with Walgreens by calling 937-254-2156. Students may also use the Walgreens app to provide insurance and payment information.

You also may fill prescriptions to any local pharmacy.
What if an outside physician orders labs for me?
For patients with a lab order from an outside physician, the Student Health Center will draw blood and send it to CompuNet Labs. To have blood drawn, the patient should call to schedule a nurse appointment. Students should bring their lab order and insurance information to the visit. Please be aware that certain lab tests must be drawn at CompuNet.
The lab order must include:
- Patient name and date of birth
- If a standing order, please include frequency and duration of order
- Fasting or non-fasting
- Diagnosis code (ICD-10) MUST be included (It is not sufficient to have the diagnosis name alone)
- Specify if the results are to be mailed or faxed to the physician’s office (Please make sure that the physician's fax number and complete address is included.)
- Ordering physician’s NPI and UPIN numbers are required
What can my son or daughter do if they're sick after office hours?
When the Student Health Center is closed, there are a number of options:
- For minor medical problems, the Student Health Center Porches website has a self-care page that provides basic symptom care info.
- For minor urgent medical problems, you can schedule online to see a Premier Student After Hours Clinic nurse practitioner or physician assistant. You must bring your University of Dayton Student ID and insurance card. Premier can only see students 18 years and older
- Many insurance companies offer a "telehealth" option, so your student can talk to a provider remotely if your insurance covers this.
- For emergency symptoms (difficult breathing, severe pain, uncontrolled bleeding, etc.), students should call UD Public Safety at 937-229-2121 or 911 from a campus phone. Public Safety runs a fully-trained student EMS service 24/7 during the regular academic sessions.
What if my son or daughter is taken to the hospital? How would I know if they're ok?

If a student is transported by UD's Public Safety, the dean of students' office is notified. If they are released from the emergency department, the dean's office sends a follow-up email to the student and parent the next business day. If the student is admitted to the hospital, the dean of students' office will contact the emergency contact via phone.

Your son or daughter should make sure to include their correct emergency contact information (name, relationship, phone, and email) in their phone's emergency app, as well as noting this information when they fill out their housing contract (contact housing@udayton.edu to update info) or in Porches on their Flyer Student Services tab.
