Health & Lifestyle Program: Enhancing Health and Well-Being after a Brain Injury is a referral-only program through healthcare providers.
Purpose
The purpose of the program is to help individuals with brain injury adopt and maintain a healthy and active life following discharge from formal medical and rehabilitation services. The program assists clients for a limited time to facilitate an effective transition to community-based activities.
Program Steps
Program Steps:
- A qualified healthcare provider identifies a potential client.
- The provider completes the referral form based on the client's needs.
- The faculty coordinator notifies and recruits an appropriate student to work with the client.
- The student contacts the clinician to discuss the specific needs and goals of the client.
- The student will work with the client for up to 5 sessions.
- The student documents each session and provides a summary report to the clinician and faculty coordinator.
Eligibility
Who is eligible to participate in the program?
Individuals with brain injury who demonstrate the potential to safely participate in community-based activities with limited supervision or with the assistance of a trained family member/caregiver.
Location
The program will be offered in conjunction with our community partners depending on the client's needs. This could take place in the Dayton YMCAs, University of Dayton, or Kettering College.
