Chosen Name
- Your record must reflect your legal name. However, you are welcome to also provide your chosen name, which would then be displayed in all university systems and utilized for any mailings.
- Please visit the Chosen Name Policy for further information.
Documentation Required for Name Change
- Marriage: A copy of the marriage certificate AND a copy of a driver's license or passport in the married name.
- Divorce: A copy of the divorce decree, court entry, or court order authorizing legal name change AND a copy of a driver's license or passport in the new name.
- Legal name change: A copy of the court order authorizing the name change AND a copy of a driver's license or passport in the new name.
- Naturalization/citizenship: A copy of the naturalization certificate and/or a copy of the court order authorizing the name change AND a copy of a driver's license or passport in the new name.
- Correction due to misspelling: A copy of the driver's license or passport with the correct spelling of the name.
Documentation Required for Legal Sex Change
- A copy of the birth certificate, driver's license, federal/state photo ID, passport, or court order.
