Duplicate W-2 Request Form
Use this form to request a duplicate W-2. Please note that we do not send W-2 copies by email attachment; we will send a link to access a copy of your W-2 via Box. Alternatively, you may choose to pick up your W-2 or have it mailed to you. For tax years prior to 2019, please contact us at payroll@udayton.edu.
