Fingerprint Services

Fingerprint services are available at the Public Safety administrative offices, for any member of the University of Dayton Community.

Where:  Fitz Hall, room 195

When:  9:00 AM to 4:00 PM Monday through Friday, except holidays.

Most background investigations require electronic submission of fingerprints.  Some require paper fingerprints, which are provided at no cost for University of Dayton community members.  You must bring the appropriate fingerprint card when requesting paper fingerprint submission. 

The cost of electronic fingerprinting is $56.25 for State of Ohio and FBI, $31.00 for State of Ohio only, and $34.25 for FBI only.  Your fees will depend on what your background investigation requires.  Depending on your situation, the cost may be borne by your school or major, service organization, or university department.  If not, the individual being fingerprinted is responsible for paying in advance at the Bursar's Office (St. Mary's Hall first floor) and must bring the original receipt when reporting for fingerprinting.

What do I need?  

Please read the appropriate paragraph below for instructions. 


-- If you are an undergraduate student of the School of Education and Health Sciences's, the school will provide you a fingerprint request form and instructions.  Please bring the completed form and your government-issued photo ID (i.e. driver's license, passport).   Because many government-issued identification documents do not include the Social Security Number, you must know the number in order to be fingerprinted.  Print the Consent to Background Check form on this page, complete the signature, and bring this completed form with you.   SOEHS will coordinate payment for your fingerprints.

-- If you are a graduate student of SOEHS, print and complete the Consent to Background Check form.  Also print the Fingerprinting Information form and complete the top block, titled SOEHS Course Requiring Fingerprinting,   Pay for the fingerprint services at the Bursar's Office (St. Mary's Hall first floor).  Bring both forms, the Bursar's Office receipt, your social security number, and government-issued identification when reporting to be fingerprinted. 


--  Print and complete the Consent to Background Check form.

--  Print the Fingerprinting Information form and complete the appropriate block (either Service Organization Requiring Fingerprinting or Employment and Licensure Requiring Fingerprinting).  If the fingerprinting is being paid for by a department, put the appropriate account number in the space provided.   A person authorized to sign for the account to be charged must sign legibly in the space provided.  If the individual being fingerprinted is paying for the service, pay at the Bursar's Office in advance.

-- Know the reason you are being fingerprinted.  We will cross-check this against a state-provided list to obtain the appropriate code, which will ensure your fingerprints are properly routed.

--  Bring the completed forms and (if paid by the individual) Bursar's Office receipt, social security number, and government-issued identification when reporting to be fingerprinted.

Call 911 from UD campus phones. Call 937-229-2121 from all other phones.

Non-campus phones and cell phones will reach the Montgomery County Regional Dispatch Center when dialing 911. Add 937-229-2121 to your cell phone for emergency calls to University of Dayton Public Safety.
