Voluntary Medical Withdrawal

Voluntary Medical Withdrawal

The Voluntary Medical Withdrawal (VMW) process is designed to support student needs when the traditional processes are not adequate.  Before submitting a VMW request, we recommend you address the following:

  • Speak with your Assistant Dean to review all your academic options.
  • Speak with Flyer Student Services if you have any questions about scholarship or financial aid impact of a VMW.

If you are considering a VMW, please contact OLR as soon as possible to schedule an appointment (937-229-2066 Option 4).  As part of this process, OLR will evaluate the information provided by you (the student) as well as the medical documentation from your provider.  The information requested below will help in the evaluation of your request; however, we may also need to speak with other campus partners before we can make a decision to move forward with the medical withdrawal process.

Making a request does not guarantee approval.  Once the review is completed, OLR will send a formal determination to your University email account.  Please remember, the medical withdrawal process does not dismiss/waive any legal, disciplinary, housing, meal plan, or other student responsibilities to the university.

Please remember, a student can withdraw from individual classes during the semester through the regular withdrawal options outlined on the Flyer Student Services page ( under Your Classes > Academic Catalog.

To request a medical withdrawal, complete all fields in this form.
