The Name Change Request Form is available online.
Emailed or faxed requests will not be accepted.
Documentation Required for Name Changes
- Marriage - A copy of the marriage certificate and a copy of a driver's license/passport in the married name
- Divorce - A copy of the divorce decree, court entry, or court order authorizing legal name change and a copy of a driver's license/passport in the new name
- Legal Name Change - A copy of the court order authorizing the name change and a copy of a driver's license/passport in the new name
- Naturalization/Citizenship - A copy of the naturalization certificate and/or a copy of the court order authorizing the name change and a copy of a driver's license/passport in the new name
- Correction due to Misspelling - A copy of the driver's license/passport with the correct spelling of the name
In-Person or Mailing: Name Change Request Form
The form must have a notary's signature and seal, certifying that you have presented appropriate documentation with your new name to the notary. Return the original notarized form in person or via mail along with the required documentation to:
