Class Attendance Report Request Form
Attendance Reports can be generated for any class that has a door access reader outside the classroom. The student will present their card upon arrival (no more than 15 minutes before class or 15 minutes after class). Reports will be available the following morning. If your class meets a few times each week, you can elect to receive your reports only once a week.
Please submit a separate form for each class.
