Reporting Injuries and Incidents
Forms for Reporting Injuries, Incidents and Near-Misses
University of Dayton employees and student employees should use the following forms to report a work related incident involving an injury or illness. These forms are submitted and managed by Human Resources:
- UD Injury and Illness Report - Employee
- UD Injury and Illness Report - Supervisor
- UD Injury and Illness Report - Witness
Employees, students, volunteers and non-University individuals should use the following form to report an incident or near-miss. Non-employees should also use this form to report incidents involving injuries (employee related injuries should be reporting using the UD Injury and Illness Report above). This information is reported to Environmental Health and Safety.
