Injections ordered by outside physicians
The Health Center will give most injections ordered by a patient’s physician, but the patient is responsible for bringing the medication to be administered and the order must include the following:
- Patient's name and date of birth
- Medication name, dosage, frequency of injection and the date of the last injection
- Reason for the injection and diagnosis code
In the case of DEPO-PROVERA, we can only administer this for a non-contraceptive condition.
If receiving certain immune suppressant medications, for example, Enbrel, Humira, Simponi, or Methotrexate, it is recommended that you have annual tuberculosis testing. The Health Center can provide this testing for you. Please call 937-229-3131 for any questions or to schedule an appointment.
